TERMS, CONDITIONS & LIABILITY RELEASE: I understand that Striker Soccer Academy assumes no responsibility for injuries or illness which I /my child might sustain whilst participating in activities of the Academy. I certify that I /my child is in good physical condition to participate. I accept that inclement weather may disrupt / cancel Academy sessions and that there would be no alteration in dues. I give permission for the Academy to use photography / film footage which may include me/ my child's image to be used solely for the promotion of the Academy. I hereby release Striker Soccer Academy, the Staff/Coaches and field property for any liability or claims for injuries or property damage arising out of my/my child's participation in this Academy. By writing my name below, I acknowledge that I understand and agree to the above waiver.
